Signing up for MyChart is easy! You will need:
- Your date of birth
- Social Security number
- MyChart activation code, if you have one
Option One - No Activation Code
If you would like to sign up and don’t have an After Visit summary from your physician or a Baptist Health facility or an activation code, you can still sign up for MyChart. If you've been a Baptist Health patient, your MyChart account will connect to your medical record.
Option Two - Activation Code
During your next visit to your physician or a Baptist Health facility, you will receive an “After Visit Summary” that contains a MyChart activation code. The code will allow you to log in and from there, create your own username and password for MyChart. Sign up for MyChart >.
Schedule an Appointment with Your Primary Care Provider
You can use MyChart to schedule appointments with your primary care provider. To schedule an appointment with your primary care provider in MyChart, follow these steps:
- Log in to MyChart
- Select the Appointments Icon
- Select the Schedule an Appointment Button
- Select the provider you are interested in scheduling an appointment with
- Select the reason for the appointment then choose a day and time
- Verify your personal information or make changes
Send a Non-Urgent Message to Your Provider
- After logging into your MyChart account, hover over the Messaging tab and select “Ask a Question”
- Select the option that applies to you: new medical question, request a medication refill or customer service question.
- If you are sending a new medical question, choose a recipient, select a subject and compose your message. Select send when you are complete. You can expect a response within two business days.
View Upcoming Appointments or Complete Medical Questionnaires
After you login, your home page will show you a to-do list that will show any of the following:
- Upcoming appointments
- Health reminders
- eCheck-In and Medical Questionnaires for upcoming appointments
Here you can complete eCheck-In and answer questionnaires up to seven days prior to your visit, saving you time when you arrive.
The “eCheck-In” activity will ask you to review and verify your personal information, just as you would if you were registering with the front desk. You will then be prompted to complete any medical questionnaires related to your visit. If at any time you cannot complete part of the process, you can cancel or tap “Finish Later” to leave the activity and save your answers. If you come back later to continue, you’ll be returned to where you left off.
Interact with other apps
You also may import information from MyChart into other applications that, with your permission, can use that data to help you manage your care, medications, nutrition and more.
For example, Baptist Health supports the Apple Health Records app on iPhone, which makes it easy for patients to see their available medical data from multiple providers whenever they choose.
With Apple Health Records, patients can have medical information from participating healthcare institutions organized into one view, covering allergies, conditions, immunizations, lab results, medications, procedures and vitals, and will receive notifications when their data is updated. Health Records data is encrypted and protected with the user’s iPhone passcode, Touch ID or Face ID.

Information is MyChart is the same as what is on file with your providers. You can correct this information at your next appointment; if you are concerned and want to change the information sooner, contact your provider's office.
You will generally receive an answer within 1-2 business days. Please note that MyChart should not be used for urgent situations. Please contact your provider's office by phone if the situation requires immediate attention. If you are experiencing an emergency, call 911 or go to the nearest emergency room.
Yes. This is called “proxy access” and allows an adult to access their family member's MyChart. This proxy access can be for minor children (under 14), or for an adult who has designated someone as a health care decision maker. A Proxy Consent Form can be completed in-person at the provider's office.
No. Medical information is specific and unique to every individual. Each person should have their own MyChart account. To ask questions for someone else, you must have proxy access to another person’s MyChart account, and then use their account to ask questions.
No. Because medical information is specific and unique to every individual, each adult must have their own individual MyChart account. People may grant proxy access to their spouse (or another adult) by contacting their provider's office and completing a Proxy Consent Form.
You may click the "Forgot Username" or "Forgot Password" link on the sign-in page to reset your password online. You may also call our support line Monday through Friday from 9am to 5pm at 502-253-4820 or 1-844-764-7820 to request a new, secure password.
First, log into MyChart. To update your password or security questions, select Settings at the top, and then select Security Settings. To update your personal information, choose the Health Folder at the top, then select Demographics.
Access to your account is controlled through secure activation codes, personal usernames and passwords. Each person controls their password, and the account cannot be accessed without that password. You will never receive an email containing health information or other personal details, as this information is only viewable while logged in to you account. You will also never receive a email asking for personal information.
MyChart is owned and operated by Epic, the most widely used electronic medical record. Epic is fully compliant with federal and state laws related to patient privacy. Your name and e-mail address will be treated with the same care and privacy that given your to your health records and will never be sold or leased.
You have two ways to bring your health information together—combined view under your Deaconess MyChart or separate views of your other MyChart accounts from other healthcare organizations.
How To View Your Healthcare Information:
Combined View Under Deaconess MyChart
- Log into your MyChart portal and in the Sharing section under the Menu, use the Link My Accounts to add all of the organizations where you’ve received care
- This will pull records from other organizations into the Deaconess instance of Epic for a single view of all records from one MyChart account
- Please note—this is not a complete import and the records are not actionable (you can’t request a refill on a medication from another organization’s records)
Separate Views of MyChart
- Log into your MyChart portal and select the organization you want to view of MyChart OR
- Use the Lucy menu and click Go to MyChart Central button. MyChartCentral allows you to link all of your MyChart accounts from different organizations and then you can toggle between them with a single login.
- You do not need to change the checkboxes on the Lucy and MyChartCentral page regarding health information and sending demographic information.
